Start Page:
  • Name: Enter all or part of the name of the organization or individual provider. Since agency names change frequently, this is less reliable than the phone or email option.
  • Phone: Enter the area code, 3 digit, and 4 digit values for a phone. This can also include toll-free numbers.
  • Email: Enter all or part of the email address.

Edit Page - Part 1:
  • Type: Select the type of resource. If you are filling out the form for a private provider, select person. Otherwise, select organization..
  • Name: Enter the complete name of the resource. For persons, enter their first name and then last name.
  • County: Enter in the county in which this resource is physically located. If you are adding a web site or other resource that does not have a physical location, please enter web.

Edit Page - Part 2: Mailing Address
  • Enter in the mailing address for this resource. If your address needs more than two lines combine the information on the second line.

Edit Page - Part 3: Physical Address
  • Note: Enter information for this section if your mailing address is different than your physical location (so someone could drive to your office for services)

Edit Page - Part 4: Phone Information
  • Phones: All phone information is broken into 3 boxes: area code, 3-digit, 4-digit. When entering phone values, do not add "(", ")", or "-"; just provide the digits.
  • TDD: Phone for hearing-impaired clients.
  • After Hours Phone: Phone number for clients to call in an emergency situation.

Edit Page - Part 5: Internet Information
  • Enter the main email for an orgranization or service provider (only 1 email please). For the web page, there is no need to provide the "http://" as part of the address.

Edit Page - Part 6: In-Depth Information Part A
  • Description: Provide a brief description on your organization and the services it provides..
  • Ages Served: Please list the range of ages served by your resource. If all, enter all.
  • Service Hours : Enter the hours that your resource is available for clients.
  • Fee: Please check any/all of the following fee options.
  • Elgibility Requirements: Please describe any elgibility requirements your resource has in providing its services.

Edit Page - Part 6: In-Depth Information Part B - Categories
  • Please review each category and put a check in the box if it represents a service that your resource provides. You can check off numerous boxes but should have at least one box checked as this will be used to help find your resource on from the search page.

Edit Page - Part 6: In-Depth Information Part C - Locations Served
  • Please review each location and put a check in the box if it represents an service area that your resource covers. You can check off numerous boxes but should have at least one box checked as this will be used to help find your resource on from the search page.
